Wellness Discount Program: Wellworks For You
This program is eligible for all Groups/Bargaining Unions. (Non-Union, FOPC, FOPR, 1102, 1590 & 320) and encourages physician engagement for employee health and wellbeing.
Your completed Wellworks form is due by March 31, 2025, to receive a 2% discount for July 1, 2025, on your medical premium.
The look back period is April 1, 2024 to March 31, 2025. This means if you have an annual physical completed within that time frame you will be eligible to receive this discount effective for July 1, 2025. If your physical falls outside of that time frame, you have from now until March 31st, 2025 to get your wellness physical and blood work completed, and your form submitted to Wellworks.
Program Requirements for Full Credit:
- Fitness for Duty and DOT Physicals do not qualify for completion of the Wellworks Program.
- Only the attached Wellworks Physician Results Form will be accepted.
- The bloodwork must be completed to receive the premium discount.
- The Physician Results Form cannot be signed by the City’s Physician/Medical Dispensary.
- You cannot resubmit the Physician Results Form you used last spring to receive credit for July 1, 2025.
- Physician Results forms must be completed and submitted to Wellworks by March 31,2025 for you to receive a 2% discount for July 1, 2025. (see below on how to submit)
If You Cannot See Your Physician in the Office:
- Wilmington 1st Walk-in located on Miller Road can complete the Wellworks physical and bloodwork!
- No appointment necessary
- You must mention you are a City of Wilmington employee
- You must state the purpose of your visit is for the biometric screening
- No co-pay will be incurred as this is a preventive exam
- You will be required to complete the physical first and then go to a Labcorp site for your bloodwork. The Wilmington 1st Walk-in Physician will call you within 24-48 hours to review your results and complete your form.
- Your PCP will be able to view the results of your physical and bloodwork for follow-up.

Please submit all Physician Results forms to one of the following:
- Scan and email to: forms@wellworksforyou.com
- Upload to Portal: Click the Upload a Form tile from the homepage or via the menu page select the event title from the dropdown and upload your form to the portal. This will be securely emailed for processing. Users are limited to one (1) file per email.
- Upload to Mobile App: Take a photo of your form using your Smartphone. Next, upload it to the Wellworks For You Mobile App via the Contact Us/Send a Form tab in the menu, located in the top left corner of the home screen.
- Smart phone users can take a picture of the form and email to forms@wellworksforyou.com
Submission via email (forms@wellworksforyou.com), Wellness Portal or Smartphone App, will result in an immediate confirmation that your form was received. Any other means of submission requires you to log into your Wellness Portal or Smartphone App to confirm your form was processed.
Results Forms should NOT be sent to the Employee Benefits Division.
